Online Registration Now Open

2024-2025 Online Registration Instructions

These instructions can be used to register any new student, including new Kindergarten students.

  • If you reside OUTSIDE (non-resident) of Springfield Local School District, you must first complete an Open Enrollment Application.

    • Once completed, the Open Enrollment Application must be e-mailed to sp_grubbs@springfieldspartans.org
    • Your application will be returned to your email once it has been approved or denied.
      • Instructions will be included to continue registration with your approved application.
  • If you reside INSIDE (resident) of Springfield Local School District:

    • Use Final Forms to complete your new student registration.


      • Add a New Parent Account (Instructions)

        • If you already have a Final Forms account that you use with an existing student, you will utilize your existing account.
      • Register a New Student (Instructions)
  • The following documents must be uploaded to Final Forms in order to complete enrollment:
    • Required Documents

      • Parent/Guardian Driver's License or State Issued ID Card - Must be presented at registration appointment for proof of identity.
      • Birth Certificate
      • Immunization Records
      • Report Card or Transcript from last school attended
      • Custody Paper--the person enrolling a student must show proof of custody. If there has been a divorce or any other circumstances for which custody papers are required. If no legal papers are available, the person enrolling the student may contact the superintendent's office for assistance.
      • Proof of Residency -
        Must provide (1) proof from the following list:
        • Copy of rental agreement
        • Copy of proof of purchase of home
        • Tax statement or tax return with address in district
        • Property tax payment receipt
        • Home being built in district, provide letter from builder (this proof alone is all that is necessary).
        Only legal residents of the district may enroll. Misrepresentation may result in prosecution.
      • Must also provide two (2) proofs from the following list:
        • Ohio Work's identification card with district address
        • Copy of utility bill (gas or electric) or copy of utility deposit
        • Certified voter registration document with district address
        ANYONE WHO IS NOT ABLE TO MEET THESE RESIDENCY REQUIREMENTS SHOULD BE REFERRED TO THE OFFICE OF THE SUPERINTENDENT.

      Special Situations

      • Custody Decree - (if applicable) Complete, current legal documentation must be presented at the time of registration. Notify registrar of any future revisions.
      • Guardianship - (if applicable) Complete, current legal court documents must be presented at the time of registration.
      • Special Education Paperwork - (if applicable) The current ETR/MFE and IEP for students with disabilities, or the current Section 504 Accommodation Plan, or Gifted Identification information must be presented at the time of registration, if applicable. An official request will be made to the previous school for these documents as appropriate; however, having the documents upfront facilitates a smoother transition.
