Statement of Residency
Along with the Sworn Statement you will need to have the following:
Property Owner or listed tenant must provide
• Property Taxes/Mortgage statement
• Lease/rental agreement along with a statement from the leasing department or landlord with the knowledge of new tenant.Tenant must provide two additional documents as proof of residency
•W-2 with name and address
• Post Office change of address
• Job and Family Services dated with name and address , etc
• Voter RegistrationPlease note, Sworn Statements:
1) Are only good for one year from date signed
2 ) We will no longer except Driver’s License as a proof of residency
